Our staff at Advanced Behavioral Wellness is excited to help you obtain your medical marijuana card pursuant to Senate Bill 47. Our goal is to make the process as easy and quick as possible for you. Our staff will guide you through the entire process from scheduling your appointment to receiving your medical marijuana card.
Application Process
To begin your process, you must schedule a visit with a medical cannabis provider to obtain a written certification for the registry ID card. Your initial examination must be in person.
Following your evaluation, you must apply online through the patient and caregiver registry portal and provide:
- Caregiver information, if applicable
- Written Certification
- Signed and notarized signature form
- Application fee (this is separate from evaluation fee charged at Advanced Behavioral Wellness)
The Cabinet will approve or deny within 30 days of application being completed. If approved, you will receive a digital registry card that must be printed or saved on your mobile device.
A cardholder must purchase medical cannabis at a licensed dispensary.
Qualifications for Applicants
- Resident of Kentucky
- At least 18 years old
- Diagnosed with a qualifying medical condition and has obtained a written certification from a medical cannabis provider
- Must not have any disqualifying felony convictions
Qualifying Medical Conditions
- Any type or form of cancer regardless of age
- Chronic, severe, intractable, or debilitating pain
- Epilepsy or other intractable seizure disorder
- Multiple sclerosis, muscle spasms or spasticity
- Chronic nausea or cyclical vomiting syndrome
- Post-Traumatic Stress Disorder

1. What will happen during my medical evaluation?
During your evaluation, a state licensed provider will evaluate and certify you for marijuana use. During the evaluation, you will discuss medical history as well as be counseled on the benefits and risk of THC and other cannabinoids. You will be given a urine drug screen and discuss all the other medications you are currently taking (prescription and over the counter). A drug monitoring report will be pulled to verify medications.
2. What will I need to bring to my first visit?
On your initial visit, you will need to bring your medication list, medical records for diagnosis if other than PTSD (these can also be obtained during visit, if you have access online to records), driver’s license and insurance cards.
3. How often will I have to be seen?
You must be seen for an initial visit and then for a one-month follow-up. This follow-up visit is to ensure that card had been received, and everything is completed and going well. You will then return at the end of your year.
4. How long is my card good for?
A Kentucky medical marijuana card is good for one year. Your card can be revoked should your felony status change or other disqualifying factors come into play.
5. Will my medical insurance cover any of the cost?
No, insurance policies are not covering medical marijuana currently.
6. Are all felonies considered to be disqualifying?
A disqualifying felony is on in which the person is classified by the Department of Corrections as a violent offender under KRS 439.3401 or a violation of a state or federal controlled substance law classified as a felony in the jurisdiction where the person was convicted. However, there are exceptions for individuals with felony drug convictions.
